The Summer Meals Parent/Caregiver Survey was originally run on Qualtrics, but we created this electronic version of the survey to showcase the questions that were asked.
Why? Oregon Department of Education, in partnership with Oregon State University (OSU) Food Hero, wants to create videos for kids and families that make learning about fruits and vegetables and how they are grown fun and interesting. We need your help! Your responses to these surveys will tell us how we can improve the next videos we make.
Who can take these? Elementary and middle school-aged students, siblings, parents, and caregivers. Parents and caregivers can do these side-by-side with your youth and young children.
What? Click each link below to watch a short, 2 - 3 minute video on a OH4S featured food, and answer questions about what you saw! Each survey will take up to 10 minutes to complete. Be sure to complete all of the questions and click the arrows >> at the end of each page to fully submit your responses. Please ask an adult for help if you need it! Parents can coach you through each question and help you submit your responses.
When? You can re-use each link if you need to go back in to finish a survey. Take as many as you like (up to 8 videos), but complete one survey per topic for each person, please.
Where? Take these at home and share the links with your family and friends! Each link connects to an online survey. If you would prefer to complete the questions on a paper survey, (a PDF file), please email food.hero@oregonstate.edu and indicate "OH4S youth video feedback" in the subject line to request a paper survey format.

Unit Goals for FFY 2024 - 2025: Administer/collect (20 - 30 per unit) What's Cooking Surveys with adult audiences (English or Spanish). This survey will run continuously during FFY 2024 and 2025.
The What’s Cooking? survey builds on questions from the Family Dinner and Cooking Tools Surveys, both of which were field-tested for validity. The WC survey was field-tested with SNAP-Ed educators in December of 2019, and further refined with educator feedback and the following criteria in mind:
1) applies best practice concepts from 12+ years of Food Hero research; 2) tested and approved by Multnomah county food pantry participants (FDS + CTS) or SNAP-Ed educators (What’s Cooking?); 3) designed to be short and easy to comprehend; and 4) enhanced with photos to aid with comprehension.
This is a placeholder. You should be redirected to foodhero.org/celebrations.
The Food Hero Bingo project was created to promote comprehensive wellness for children and their families over outside of school. The card includes 24 tasks covering healthy eating (14), physical activity (7), and less screen time (3). All tasks were designed to be achievable and replicable by most families, and desirable to be reported by schools as collective successes.

Kids' Tasting surveys and voting with counting chips help identify recipes that have been tested and approved by at least 25 kids. “Kid Approved” means that at least 70% of the kids who tried a recipe in a taste test "Liked!" the taste.

For FFY 2024 - 2025, please continue to use the Food Hero Evaluation Cover Sheet with all Kids Tasting assessments (paper survey and disc voting). Note the new field on the Cover Sheet to list any garden ingredients used in the recipe and describe if participants/students helped to grow, tend, or harvest any of the garden produce used in the recipe they tasted.
You can be creative and showcase your results any way! Additionally, Food Hero created the following forms to use!
